1 - I understand that Gloria de Gast is an Holistic Practitioner, Life Counselor, Massage Practitioner, Energy Healer and Trainer, not a medical doctor or psychology professionals, she will therefore practice according to her degrees, nor is she claiming to have any special credentials of any kind.

Accordingly, any information or ideas they share during our work together is only a personal opinion and will not be construed as medical or psychological advice, professional financial advisory services, legal advice or anything else other than their personal opinion and intuitive guidance. I further understand that if I have a medical or psychological condition, it is advisable to seek medical or psychological advice from a licensed professional in those fields.

I understand that the massage/bodywork/release work I will receive or learn is provided for the basic purpose of relief from stress and muscular tension. If I experience any pain or discomfort during the session or workshop, I will immediately inform the practitioner/trainer so that pressure or strokes may be adjusted to my level of comfort. I further understand that massage/bodywork/release should not be considered a substitute for medical examination, diagnosis, or treatment and that I should see a physician or other qualified health care specialist for any mental or physical ailment of which I am aware. I understand that massage/bodywork practitioners/trainers are not qualified to diagnose, prescribe, or treat any physical or mental illness, and that nothing said in the course of a session should be considered as such.

2. In consideration of the services, information, and support I have received or will hereafter receive from Gloria de Gast I hereby hold harmless mrs de Gast-Gagliardo, and any related entities from any and all liability in consequence of such services, information and support given, and I release and waive all claim for damage, as a result of such services, information and support. This release shall be effective and binding upon my heirs, next of kin, executors, administrators and assigned. I have read this waiver & release in its entirety before agreeing to it, and I understand its effect. I understand that I am waiving certain legal rights that I or my heirs, next of kin, executors or administrators otherwise may have had.

Because massage/bodywork should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions and have answered all questions honestly. I agree to keep the practitioner/trainer informed of any changes to the above profile and understand that there shall be no liability on the practitioner‘s part should I fail to do so. I also understand that any illicit or sexually suggestive remarks or advances made by me will result in immediate termination of the session/workshop, and I will be liable for payment for the full time scheduled. I agree to honor the cancellation policy or else be responsible for payment of 100% of the fee that would have been due.

3. I understand that the classes, training, facilitations, live calls and events, question and answer sessions, information, materials, methods, techniques, frameworks, worksheets, attachments, recordings and all related content taught and shared by Gloria de Gast in any format or setting (hereafter referred to as “the information”) are designed for my own educational purposes only. I understand that “the information” contains copyrighted material and may include trade secrets and other intellectual property that is the sole property of Gloria de Gast, and that without specific written permission from Gloria de Gast-Gagliardo “the information” may not be reproduced in any manner in whole or in part and may not be shared in whole or in part in digital or non-digital format, in person or over the phone or in any other way with anyone else or in any public or private forums or groups now or in the future. I further agree not to use any of “the information”, with other people or private clients unless I have specifically completed specific trainings and gained permissions.

I agree not to share any writings about or summaries of any of “the information” or teach any part of it to others in a group program or privately one on one or in any other setting or via any other means without written permission from Gloria de Gast-Gagliardo.

4. The following sometimes occur during massage. They are normal responses to relaxation and/or touch, and need not be embarrassed nor suppress them. Movement or release of intestinal gas - crying - laughing - strong emotions - sighing - groaning - yawning - softening of muscle tissue - cognitive or felt memories - stomach gurgling - need to move or change position. At any time during your session/workshop please let the teacher/trainer or therapist know if there is anything to be done in order to help you feel more comfortable.

5. I understand that if I purchase one or more private sessions with Gloria that I need to provide at least 24 hours notice to reschedule. I agree that if I don’t reschedule with at least 24 hours notice or if I show up more than 5 minutes late for my session, it will be considered a no show, and I will lose my session and will not receive a full or partial refund for the session.

6. I declare that the information I have given is correct and promise to notify the Therapist/trainer should there be any changes to my health. As far as I am aware I can undertake treatment without any adverse affects. I have been fully informed about any contraindications and am willing to proceed with the treatment.

I have informed the massage therapist/trainer of all my known physical conditions, medical conditions and medications, and I will keep the massage therapist updated on any changes in medications, and that spinal manipulations are not part of massage therapy.medications, and that spinal manipulations are not part of massage therapy.
